Tools for care wishes with Carpenter Hospice Social Workers
Many people don’t know how to begin a conversation about their preferences for medical care. Advanced care planning (ACP) conversations help people maintain some control over how they are cared for, what treatments they receive, where they die, and who accompanies them through the dying process.
In this interactive session, learn how to begin conversations with your Substitute Decision Maker, and other loved ones, about your values, wishes, and preferences for care. This session will be facilitated by Dr. Nora Curran-Blaney and registered nurse Ruth Forbes and is open to all community members.
